If you're a military retiree or the spouse of one, you probably already know that TRICARE for Life (TFL) is one of the best secondary coverage programs in the country. As long as you have Medicare Part A and Part B, TFL acts as your secondary payer — covering most of what Medicare doesn't, with no monthly premium. It's a benefit you earned through your service, and it's worth protecting. But here's the part many veterans miss: having TRICARE for Life does not prevent you from also enrolling in a Medicare Advantage plan, and doing so could put real money back in your pocket.
The opportunity: the Part B giveback. Some Medicare Advantage plans offer what's called a Part B premium giveback (also called the Part B reduction). Instead of the plan charging a monthly premium, it actually returns part of your Medicare Part B premium — often $30 to $170 per month or more, depending on your county — directly to your Social Security check. For a retiree couple where both are eligible, that can add up to thousands of dollars a year in found money. And because TRICARE for Life still pays as your secondary after Medicare, your core protection doesn't go away.
How the stacking works. When you enroll in a Medicare Advantage plan, that plan becomes your primary Medicare coverage in place of Original Medicare. TRICARE for Life then pays second, just as it always has — it coordinates with whichever form of Medicare you carry. That means the hospital, medical, and emergency benefits you rely on through TFL continue to work. The Advantage plan simply replaces Original Medicare as the first payer, and in many cases it layers on extras like dental, vision, hearing, over-the-counter allowances, and fitness benefits that TFL alone does not cover.
A common strategy worth considering: enroll in a Medicare Advantage plan WITHOUT the built-in Part D prescription drug coverage. Why? Because TRICARE for Life already includes robust prescription drug coverage through the TRICARE Pharmacy Program, which by law acts as 'creditable coverage' — meaning it's at least as good as Medicare Part D. Adding a Part D plan on top of TFL is usually redundant and can even cause you to lose TRICARE pharmacy benefits while you're enrolled in Part D. By choosing a Medicare Advantage plan that does not include drug coverage (sometimes called a 'MA-only' or 'no-PDP' plan), you keep your TRICARE pharmacy benefits intact AND capture the Part B giveback money.
The honest pros of adding a Medicare Advantage plan to TRICARE for Life:
• Part B giveback can return hundreds to thousands of dollars per year to your Social Security check.
• Extra benefits TFL doesn't cover — dental, vision, hearing, OTC allowances, gym memberships, and transportation.
• $0 or low monthly plan premiums in many areas, so the giveback is nearly pure savings.
• Your TRICARE for Life secondary protection stays in place, so your core coverage is not weakened.
The honest cons and cautions to weigh:
• Medicare Advantage plans use networks, prior authorizations, and referrals — less freedom than Original Medicare + TFL, which lets you see any Medicare provider nationwide.
• If you travel frequently or live part of the year in another state, confirm the plan's service area covers both locations.
• Maximum out-of-pocket limits on some Advantage plans have risen sharply for 2027 — read the Annual Notice of Change carefully.
• You must continue paying your Medicare Part B premium (the giveback reduces it, it doesn't eliminate it unless the plan covers the full amount).
• If you later want to return to Original Medicare + a Medicare Supplement (Medigap) plan, you may face medical underwriting outside your initial enrollment window.
Who this strategy fits best. Military retirees who want to lower their out-of-pocket healthcare costs without losing TRICARE for Life, couples looking to capture the Part B giveback as extra retirement income, and veterans who value the added dental, vision, and wellness benefits. It tends to be a strong fit for retirees who are comfortable with network-based care and who want predictable savings.
Who should think twice. Veterans who see out-of-network specialists frequently, who travel or live internationally for long stretches, or who want the absolute freedom of Original Medicare with no prior authorizations may prefer to keep Original Medicare + TRICARE for Life and skip the Advantage plan. There's no single right answer — it depends on your doctors, your budget, and how you use care.
